I'm located in Suite 5B, the door to access my clinic is in the courtyard. If you arrive early, please wait in your car or in the chairs right outside suite 5. I will come to the door and invite you in to the clinic. You may text when you arrive to (501)301-0441

The easiest place to park is to turn onto N. Grant (heading south) between Skin Fix and the parking lot with the painted fence with a heart. There is no assigned parking.

If you are handicapped and need an appointment in person, please ensure you contact us via email at least 48 hours in advance. Your appointment will still be at Grant Place, but in another suite close to handicapped parking. You will enter on the alley side of the building for your appointment. More instructions will be given when we confirm your appointment.

WHAT TYPE OF INSURANCES DO YOU ACCEPT?
At this time, my clinic accepts Aetna, Arkansas Blue Cross and Blue Shield, Cigna/Evernorth, Web TPA, Ambetter Commercial, United Health Care, UMR, Care Credit, and self-pay. I am working to credential with Municipal Health Benefit. If you don't see your insurance listed, let me know. I will continue to update this site when I am in the network with insurers.

Arkansas State Board of Nursing referrals must follow all board rules and pay full amount at the time of the appointment. Please email us at admin@pulaskiheightswellnessclinicpllc.com so we set up your appointment appropriately. These appointments are $650 and are 2 hours in length. Your report will be emailed directly to the board, and you can request a copy of it from the Arkansas State Board of Nursing. All paperwork must be completed at least 24 hours ahead of the appointment.
I do not accept Medicare or Medicaid.
I offer super bills to patients to submit to their insurance carriers, in some cases your insurance will offer to reimbursement your out of pocket costs. Please check with your insurance on how to submit a super bill.

Refunds are not issued. Missed appointment fees apply. More details in intake paperwork. Intake paperwork must be completed 24 hours prior to appointment or your appointment will be rescheduled. All paperwork must be completed via the client portal.

As a nurse practitioner, I am well versed with confidentiality and laws protecting your protected health information. I adhere to strict ethics as a provider. Your intake paperwork will have more details and specifics about working together, and we will also go over this at your intake appointment. A trusting relationship is the key to growth and healing, and I'm honored that you chose to share your life with me.
